Jackson Health System offers palliative care for adult and pediatric patients. We adhere to the concept of "patient first" to accommodate the patient's individual needs and promote a sense of wholeness and well-being.

Jackson Memorial Hospital

In keeping with Jackson Health System’s mission to provide the highest standard of care to all our patients, Jackson Memorial Hospital created a special Palliative Care Unit that promotes comfort and care for adult patients and their families. Palliative care is the comprehensive care and management of the physical, psychological, emotional and spiritual needs of patients and their families with chronic, debilitating or life-threatening illness. The program emphasizes the importance of education and communication with patients and families to help them have a better understanding of the underlying disease process.

The palliative care team is available 24/7 to provide timely access to inpatient palliative care and consultation services.

 

The Palliative Care Team

Our staff members include physicians, registered nurses, nurse practitioners, social workers, case managers and clergy. Our team identifies the immediate and long-term goals of care and promotes advance care planning. We continue to optimize symptom control and improve a patient’s functional status in order to achieve their maximum quality of life.

The Palliative Care team also:

  • Serves as educators and mentors for staff
  • Promotes timely access to palliative care services
  • Collaborates with primary care professionals  in developing a plan of care
  • Provides physical, psychological, social and spiritual support to patient and family
  • Facilitates care planning with family to meet multidisciplinary care needs caused by life-limiting illness
  • Facilitates patient understanding of diagnosis and prognosis to promote informed choices
  • Assists patients in establishing goals of care and establishing priorities
  • Encourage advanced care planning.

Jackson Memorial Long Term Care Center

Jackson Memorial Long Term Care Center (JMLTCC) is a non-profit, 180-bed skilled nursing facility licensed by the state of Florida. JMLTCC is one of two skilled nursing facilities owned and operated by Miami-Dade County, Public Health Trust and Jackson Health System to assist Jackson Memorial Hospital with the timely discharge of patients who cannot be easily discharged from the hospital to home or other community nursing homes due to:

  • Complex Medical Conditions
  • Special or Complex Psychological Needs
  • Psychiatric or Behavioral Issues
  • Lack of funding from Medicaid or Medicare
  • Young Age
  • History of Substance Abuse
  • Involvement with Criminal Justice System
  • Immigration Status
  • Do not qualify for Medicaid or Medicare

  • HIV Care - JMLTCC was the first nursing home in the United States to admit an AIDS patient in 1981 and has maintained an active HIV/AIDS program ever since.

24-hour Medical Staff

JMLTCC has 24-hour skilled nursing care provided by an interdisciplinary team of physicians, registered nurses, licensed practical nurses, dietitians, therapists, social workers, pharmacists, certified nursing assistants and support staff. All members of our professional staff are duly licensed and/or certified. Our staff reflects the diversity of our resident population and is specially trained and/or licensed in adult medicine, geriatrics, rehabilitation nursing and restorative nursing.

Jackson Memorial Perdue Medical Center

Jackson Memorial Perdue Medical Center is a long-term-care facility located on a beautiful 11-acre campus in South Dade. The medical center is a licensed, 163-bed skilled nursing and rehabilitation facility. We provide care for a diverse cultural population of younger and older adults. Our residents have full use of Jackson Health System and University of Miami Miller School of Medicine clinics, specialists and inpatient services.

(Behavior Enrichment Structured Therapy) program, which meets five and one-half days per week and is managed by a clinical social worker and two certified nursing assistants.

24-hour Medical Staff

Jackson Memorial Perdue Medical Center provides 24-hour medical and skilled nursing care provided by staff physicians, registered nurses, licensed practical nurses, dieticians and certified nursing assistants. Staff members are specially trained and/or licensed in adult medicine, geriatrics, rehabilitation nursing and restorative nursing, and all members of our professional staff are duly licensed and/or certified.
